SAY San Diego’s Neighborhood Networks are for youth and adults with chronic health conditions, or whose health plans determine a necessity for further care coordination. We’re committed to helping you navigate any complex health and social needs, so that your overall well-being is rightfully tended to.
Through the connections of our Health & Housing Support, you’ll receive direct case management and additional community resources to keep you on track to true health and wellness.
