POSITION PROFILE:
The Case Manager is responsible for the development and facilitation of socio-emotional programming for Real Connections and Field Healthy Start school-based resource centers. The Case Manager will be experienced in direct client services, group facilitation, family support services, multi-disciplinary case management, and community development. This position requires independent judgment, creativity, and adaptability.
ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:
- Facilitate socio-emotional groups for elementary, middle, and high school students
- Provide one-on-one socio-emotional support utilizing Cognitive Behavioral Therapy-based curriculum.
- Develop and coordinate youth leadership trainings for Youth Ambassadors.
- Collaborate with youth to plan, develop, and implement mental and behavioral health awareness campaigns.
- Support the outreach, recruitment, and retention of program participants
- Provide care coordination and case management services maintaining accurate documentation of client files as well as accurate and timely entries into a client tracking database
- Develop and maintain positive relationships with students, families, school staff, and community organizations
- Attend relevant community, school site and district meetings

QUALIFICATIONS (Education, Experience, and Certifications):
- Bachelor’s Degree in Social Work, Child and Youth Development, Humans Services, or related field preferred
- Experience working with diverse populations with varying backgrounds
- Experience working with school systems is highly desirable
- Experience in direct client service, particularly experience with youth and families, and community development
- Experience facilitating groups, case management, and delivering curriculum-based interventions.
POSITION PROFILE:
The Community Organizer is responsible for the development and facilitation of parent and youth alcohol, tobacco, and other drug (ATOD) prevention and education efforts as part of the San Diego Promise Neighborhood in Barrio Logan and Logan Heights.
QUALIFICATIONS (Education, Experience and Certifications):
- Bachelor’s Degree preferred or working towards one.
- Bilingual in English and Spanish is preferred.
- Experience in community and economic development working with collaborative efforts and/or similar initiatives relevant to ATOD environmental prevention.
- Experience in working with adolescents on ATOD prevention activities, and/or leadership development.
- Must be able to provide proof of COVID-19 vaccination

POSITION PROFILE:
This is a professional position requiring the use of independent judgment and individual discretion under direct supervision of Vice President of Child and Youth Development. The Program Director position is responsible for monitoring and supervising the daily operations of programs in the CYD Unit. Primary responsibilities are to provide management and supervision to before and after school programs and vacation programs in the areas of program and staff development.
QUALIFICATIONS (Education, Experience and Certifications):
- Bachelor’s Degree in Child Development, Elementary Education, Recreation or related field or commensurate experience.
- Five years of experience supervising before and after school programs for youth 5-14 years of age.
- Three years of experience in supervisory position monitoring multiple youth programs with increasing responsibilities.
- Current Pediatric CPR, First Aid, Preventative Health, and TB certifications.

Neighborhood Navigators are community-based case managers providing intensive case management support to community members through intensive coordination of health and health-related services via Enhanced Care Management (ECM). The Housing Navigator provides housing related community supports to clients with identified housing needs through coordination with Neighborhood Navigators and ECM services. Main duties include conducting personal health assessments, guiding individuals in developing personal healthcare plans and goals, and connecting individuals to resources and support in the community, all with the goal of improving overall health outcomes.

Under the Supervision of SAY San Diego’s Dads Corps Program Director and Program Manager, the Employment Case Manager oversees the job development referrals made by Dads Corps Case Manager Team. The Employment Case Manager will have experience with employment services, education and training, vocational rehabilitation, and/or equivalent experience. The Employment Case Manager will serve as a community broker of employer relationships to create employment opportunities with varying industries and will also be called onto support clients as they prepare for employment by educating, assessing, and preparing participants for potential job applications, job interviews, and job retention.

- Bachelor’s Degree in Social Work and/or related Social Science, Associate’s Degree in a related field, or 2 years equivalent case management work experience

POSITION PROFILE:
The Prevention Specialist will be the lead position responsible for overseeing the implementation of the project activities, internal and external coordination, developing materials, conducting meetings and planning evaluation with epidemiologist and ongoing support to the Safeguard project and the Central Region Prevention Coalition. The Prevention Specialist conducts community outreach efforts in the Southeast area and work to reduce youth alcohol and marijuana use utilizing SAMHSA’s Strategic Prevention Framework.

POSITION PROFILE:
New program hiring for new team. Services are provided to those adults in reentry or on probation. This position will assist with assessments, group counseling, family work and goal setting. The CBT services are designed to support the client with a focus on an opportunity for a successful future. The This staff will be part of a team recognized as treatment leaders in evidence based practice utilizing Cognitive Behavioral Therapy. Referrals come directly from probation.

- Master’s Degree or higher in Social Work, Counseling, Criminal Justice, Psychology, Addictions or related field.
- Direct experience/expertise in working with clients involved in the justice system, substance abuse, gang activity, and criminality
