The Family Connection Hub (FCH) partners with families to promote nurturing and responsive relationships and environments through FREE individualized, family-driven support services, care coordination, and economic support. This program is intended to empower families to thrive in a connected community that enhances and restores nurturing and responsive relationships and environments. 

Designed to prevent future involvement in child welfare, FCH strengthens protective factors and builds family resilience, one connection at a time, by aligning the impact of existing and developing partners on multiple levels so that all children are safe, cherished, and have the necessary systems and structures to create equitable pathways to wellbeing.

Family Connection Hub Services for Families at Risk of Child Welfare Involvement Include:

The Family Connection Hub (FCH) provides compassionate, culturally responsive, and trauma-informed services to support San Diego families. We work alongside you to offer guidance, resources, prevention services, and a listening ear. Family Connection Hub services include:

  • Care Navigation & Family Wellbeing Plans: Families are paired with a dedicated Care Navigator to conduct a series of assessments, including the Protective Factors, Positive Childhood Experiences, and social determinants of health surveys. Using this information, families lead the creation of a Family Wellbeing Plan to create a series of goals focused on HOPE (Healthy Outcomes from Positive Experiences) to increase the family’s capacity in the four building blocks – Relationships, Environment, Social & Civic Engagement, and Emotional Growth – as well as to incorporate family engagement in one of the mental or parenting support services described below. 
  • Parenting Support Services: In-home education using the Healthy Families America (HFA) curriculum helps parents build strong, nurturing relationships with their children, promote healthy child development, and strengthen family functioning by reducing risk and building protective factors.
  • Mental Health Services: Access to behavioral intervention services through one of two approved, evidenced-based family-centered therapy models: Functional Family Therapy (FFT), a family intervention service for youth facing disruptive behavior, and Parent-Child Interaction Therapy (PCIT), a service for children and their parents or caregivers that aims decrease externalizing child behavior problems, increase positive parenting behaviors, and improve parent-child bonds.

Who’s Eligible for the FCH?

Families with children aged 17 years and younger living in San Diego County experiencing any significant risk factors, family instability, or safety threats are eligible to participate in this program. 

Please Note: Family Connection Hub (FCH) is only accepting referrals from the County of San Diego as Phase 1 of the program. Other parties cannot make referrals to SAY San Diego at this time.
