The Family Connection Hub (FCH) helps families build caring, supportive relationships and safe home environments. Families at the Hub receive free, personalized support, care coordination, and economic help. Our goal is to empower families to grow stronger in a connected community. The FCH helps families stay out of the child welfare system by building safety, support, and strength. We bring partners together so children can be safe, valued, and connected to the support they need to grow and thrive.

Family Connection Hub Services for Families:

We offer caring, personalized support for San Diego families. We listen, share resources, give guidance, and connect families to services that can help. Family Connection Hub services include:

• Care Navigation & Family Wellbeing Plans: Each family works with a Care Navigator who learns about the family’s strengths, needs, and goals. Together, they create a Family Wellbeing Plan to help the family build strong relationships, create safe home routines, connect with community support, support healthy emotional growth, and meet basic needs. 
• Parenting Support Services: Families can get in-home parenting support using the Healthy Families America program. This helps parents build strong bonds with their children, support healthy child growth, and make family life stronger and safer.
• Mental Health Services: Families can get support through family therapy programs. Functional Family Therapy helps youth with challenging behavior. Parent-Child Interaction Therapy helps parents and caregivers build stronger bonds with children and support positive behavior. Brief Strategic Family Therapy helps families with youth ages 6 to 18 who have, or may develop, behavior challenges. These challenges may include substance use, conduct problems, or delinquency.
